    Gang, I'm talking to a young researcher who needs some volunteers to take a short (25 minutes or so) survey on physical activity with MS. If you can spare a moment, I am sure Catherine would appreciate it. -D

    You are invited to participate in a research study entitled “Correlates of physical activity in a sample of adults with Multiple Sclerosis.” conducted by Catherine Pollard, graduate student, and Dr. Kristina Karvinen, Associate Professor, from the School of Physical and Health Education at Nipissing University. The results of this study will contribute to graduate student research.     The purpose and objectives of this study are to explore relationships between social cognitive factors and physical activity participation in adults with Multiple Sclerosis.

    If you volunteer to participate in this study, we will ask you to complete one online survey. The survey will take approximately 25 to 30 minutes to complete. If you are interested, you may request the researcher to provide you with the findings of the study by contacting Dr. Kristina Karvinen at kristinak@nipissingu.ca. The data will be published in research articles and presented at research conferences. There is slight risk that participation in the study may cause emotional distress since you will be asked to think about your physical activity behaviours and your diagnosis with Multiple Sclerosis. If you are worried or upset and need to speak to someone you may call the MS Society of Canada’s hotline at 1-800-268-7582 or the Multiple Sclerosis Association of America hotline at 800 532-7667 for assistance. There are no potential benefits to you for participating in the study other than knowing that you have contributed to health research which in the future may be helpful to others with Multiple Sclerosis. The information gained from this study may be used in designing future physical activity programs and educational opportunities for adults with Multiple Sclerosis.

    You will be eligible to enter a free draw for one of ten $30 VISA gift cards. If you choose to enter the draw you will be invited to provide an email address that we can use to contact you if you are found to be a winner. Your email address will be kept separate from your survey responses in order to ensure that your survey responses remain anonymous and your participation in the study is confidential. Your email address and name (if you choose to share it) will be utilized exclusively as a means of contact for the purposes of the draw. We will not share or use your email address/name for any other purpose and it will be held in strict confidence.

    Any information that is obtained from research participants in connection with this study is anonymous. Participation in this study is voluntary. You may discontinue the survey at any time. Once you submit the survey you will no longer be able to withdraw your responses. You have the right to refuse to answer any question(s) that you find objectionable or which you feel uncomfortable. Completion of this survey signifies your informed consent. Please keep a copy of this information letter for your records.
